Information for Patients with Long Term Conditions

The SWL long-term condition programme team has been working on a primary care response to Covid-19 by developing online patient information toolkits and a bespoke video, for primary care to share with patients who have the following conditions: diabetes, respiratory or cardiovascular disease.  The video and patient information toolkits can be found by clicking here

If your child is unwell and you need advice from the practice then please contact us as you usually would.  Please do not delay contacting your practice and seeking advice if you and your child need it. During the COVID19 pandemic, don’t come to the practice – contact us first.

The practice door will remain closed and ONLY patients with appointments will be given access – This is to protect the welfare of our staff and other patients.

If need to collect a prescriptions please call the practice to nominate a pharmacy and we will send this electronically to them.

We have been looking at how we can improve access to Practice services; DoctorLink is our new online service where you can access advice and appointments on line.

Registration is simple and you can register here:

For all urgent appointments, we ask that you undertake a symptom assessment through DoctorLink.

The symptom checker will ask you a number of questions to help to identify your problem.

The outcome of your assessment will direct you to the right treatment, whether emergency care, your GP, nurse or self care.

If your outcome indicates that an appointment is required, you simply click on the request appointment link.

Once the request has been submitted, you will then be contacted by the Practice Team, to arrange an appointment within the identified timeframe (as outlined in the outcome of your symptom checker).

The information you provide will be shared with our clinical team (where appropriate) and will assist in providing robust information of your problem to the required clinical team member prior to your appointment.


The practice was established almost one hundred years ago and has developed along traditional family doctor lines. The present partners have sought to continue this tradition whilst incorporating the highest standards of modern medicine. The surgery was fully refurbished in 1991 when it also became a Training Practice for aspiring GPs.

Access & Parking

The practice caters for people and patients with disabilities. There is limited provision of parking at the rear of the building. This is intended for the use by the elderly and infirm and for the use of young children. Please use with consideration.
