We welcome new patients who live in our practice catchment area. You will be required to complete a registration form and to provide photo ID (either passport or driving licence) as well as proof of address i.e. a recent utility bill or a bank statement.

We are unable to accept registrations for patients who live outside of our catchment area so please ensure you check this before completing our registration forms.
All patients who are registered at the practice can make an appointment to see any of the doctors. However when you register you can indicate who you want your ‘usual doctor’ to be. We suggest that it is better for you to see your ‘usual doctor’ if possible,since this leads to better communication, less duplication and allows the development of a good doctor /patient relationship which is very important. This is particularly true for patients with complex and long standing medical problems. It is also often beneficial for members of the same family to be treated by the same doctor. There are of course situations when you may want to see a doctor of the same sex and this will be accommodated whenever possible. Please be aware that if you request an urgent appointment, you may not always be able to see your ‘usual doctor’.
Each Patient registering MUST FULLY complete both the GMS1 Form and New Patient Information Form (below). Failure to complete both forms may delay your registration.

Cross Deep Surgery is not participating in the ‘Choice of GP Practice scheme’ therefore only patients who live within in the practice catchment area will be accepted.
Change Of Address/Details
Please let us know your new address and telephone number as soon as possible so that we can keep yours and your family’s records up to date.
If you move outside the practice area, we will no longer be able to care for you. You will need to register with a practice near to your home. If in doubt, please check this with a member of reception staff.
